Facility Evaluation Report
On 08/12/25 Licensing Program Analyst (LPA) Villegas conducted an announced visit to conduct an inspection of notification of the permeant closure received from Licensee Maxine Hartwell. LPA met with
David Berry as the reason for the visit was explained.
On 07/25/25 CCLD was notified via fax by Licensee Maxine Hartwell that as of August 6, 2025, Licensee will be closing the facility, Licensee reported South Central Los Angeles Regional Center has also been informed of the facility closure. On 08/06/25 3 clients have been relocated to C&H #7 RCFE, located at 4702 East San Vincente Street, Compton, California 90221. On 08/12/25 LPA Villegas conducted a tour of the physical plant and it was observed that all bedrooms, bathrooms, and areas in the house including the backyard were no longer occupied by clients. No clients were present within the facility.
On 08/12/25 The Department was acknowledge the closure, Licensee Maxine Hartwell will mail via USPS (Surrender) the Facility license by 08/26/25.
