Complaint Investigation Report
Allegation: Staff inappropriately pushed a client while in care It has been alleged that S-1 inappropriately pushed C-1 while in care. A special incident report was sent to this office involving C-1 pertaining to an allegation of abuse involving Staff #1 (S-1). Per incident report, on 07/22/25, C-1 was exhibiting aggressive behaviors which included grabbing, hitting and attempting to hit S-1. Per special incident report, while C-1 was being guided to the relaxation room by S-1, C-1 began to kneel and S-1 with both hands pushed C-1 to the ground causing C-1 to fall forward onto C-1’s hands and knees. Per reviewed documentation and interviews, S-1 did not utilize appropriate intervention techniques as per this program’s policy. Interviews revealed that this program has a “hands-off/hands-free” behavior management strategies policy in place. Interview and reviewed documentation revealed that S-1 placed S-1’s hands with open palms on back of C-1’s shoulders and pushed C-1 towards the relaxation room in a “guiding way” and not shoving. Allegation corroborated.
Allegation: Staff is verbally aggressive to the clients. It has been alleged that S-1 has been verbally aggressive with C-1 as well as other clients. Interviews revealed that S-1 spoke to C-1 in an authority and intimidating tone, would tell clients to “sit down” in a loud tone and at times spoke in a demeaning and unprofessional tone. Allegation corroborated.
Based on observation and interviews conducted the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED .
