Facility Evaluation Report

Licensing Program Analyst (LPA) Elizabeth Irra conducted a Case Management Visit pertaining to an incident involving Client #1 (C-1) and Staff #1 (S-1) which occurred on 07/22/25. LPA met with Noe Rodriguez and discussed the purpose of today’s visit.

During this visit, LPA obtained a copy of the staff and client rosters, staff contact information, reviewed C-1’s and S-1's files and obtained relevant documentation and obtained contact information for San Gabriel Pomona Regional Center Quality Assurance Representative. LPA interviewed the Program Director.

LPA was unable to interview C-1 as C-1 has limited communication.

A special incident report was sent to this office involving C-1 pertaining to an allegation of abuse involving Staff #1 (S-1). Per incident report, on 07/22/25, C-1 was exhibiting aggressive behaviors which included grabbing, hitting and attempting to hit S-1. Per reviewed documentation, S-1 did not utilize appropriate intervention techniques and has been placed in suspension pending further investigation.

LPA to review collected documentation and conduct interviews.
