Facility Evaluation Report
Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Noe Rodriguez and discussed the purpose of today’s visit.
This program operates Monday through Friday from 8:00 AM to 2:00 PM and provides services for the developmentally disabled clients. This facility consists of the following: (2) Vocational rooms for skill learning assignments, (1) kitchen, (1) exercise area, (1) computer room, (5) restrooms and (1) office. There are a total of (36) clients participating in this program. At the time of visit, (29) clients were present for in-person services. This program is cleared for (60) ambulatory clients (30) of which may be non-ambulatory. All clients in this program receive case management services provided by San Gabriel Pomona Regional Center.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Facility has an Infection Control Plan in place.
Physical Plant & Environment Safety: Carbon monoxide tested and operable. Fire extinguishers observed (last serviced on 03/27/25). Last drill was conducted on 03/27/25. Hot water temperature measured the following: Restroom G: 111.3*, Restroom H: 110.8*, Restroom L: 112.1* Restroom M: 111.3*, Restroom O: 112.4* and Kitchen: 111.3* Filtered drinking water is available for clients. Lockers and cubbies for staff and clients were observed. Water fountain with filtered water was observed.
