Facility Evaluation Report

Licensing Program Analyst (LPA) Gabriela Castro conducted the required annual inspection. LPA met with Leland Dalby and discussed the purpose of today’s visit.

The facility consists of the following: two (2) vocational rooms for skill-building assignments, two (2) relaxation rooms, one (1) computer room, five (5) restrooms, one (1) kitchen, and two (2) offices. At the time of the visit, thirty (30) clients were present and receiving in-person services. The program is licensed for a capacity of sixty (60) ambulatory clients, of which thirty (30) may be non-ambulatory. All clients receive case management services through the San Gabriel/Pomona Regional Center.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Physical Plant & Environment Safety: Upon entry, there is a large room with tables and chairs available for clients to sit and engage in activities. An activity calendar was observed, and clients are provided with the opportunity to choose which activities and/or outings they will participate in, as explained by the Director. The program operates Monday through Friday from 8:00 AM to 2:00 PM and provides services for developmentally disabled clients. Carbon monoxide tested and operable. Fire extinguishers observed throughout the facility and serviced. Last drill was conducted on 03/31/2026. Hot water temperatures in all restrooms were tested and found to be within the required regulatory range of 105°F to 120°F. All restroom toilets and fixtures have been upgraded to sensor-activated systems. Lockers staff and cubbies for clients were observed. Water fountain with filtered water was observed.
