Facility Evaluation Report

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit. LPA was met by Keith Colder, DSP II and explained the purpose of today's visit. At 10:30 am, Administrator, Alma Rahmaan arrived and assisted LPA. The facility is licensed to serve developmentally disabled adults ages 18-59 years old of which (4) non ambulatory only. The home is vendorized through the San Gabriel Pomona Regional Center. Currently there are (2) ambulatory clients. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Staff are adhering to infection control requirements. The facility has submitted the Infection Control Plan. The facility is a single story home consists of (5) bedrooms, of which (4) are client bedrooms and (1) is the administrator's bedroom, kitchen, dining room, living room with uncovered fireplace, (3) bathrooms, laundry area, detached garage and backyard with covered patio. LPA observed that the fireplace in the living room was not covered/secured. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Smoke alarms and carbon monoxide were tested and operable. Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. There are no firearms or weapons stored at the facility. Hot water temperature in the bathroom was measured between 105 degrees F and not more than 120 degrees F. There are no pools or large bodies of water. Backyard was inspected and it has a shaded area and sitting area. There are miscellaneous boxed items such as old light fixtures on the living room floor and some project remnants that can be safety hazards. LPA observed fire extinguisher purchased on 04/07/2026. Surety bond is valid in the amount of $1000, expires on 01/01/2027. Disaster drills are conducted on a quarterly basis, the last fire drill was conducted on 06/20/2026. *****Refer to LIC 809C for the continuation of this report. *****
