Facility Evaluation Report

Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced Case Management visit. LPA was greeted by Robert Williams and explained the purpose of the visit.

The Administrator notified LPA of the power outage on 03/10/2026 during an unannounced annual visit. LPA was informed by the Administrator, Robert Williams, that half of the facility did not have electricity due to an overload of the electrical panel . The Administrator stated the facility experienced a power outage on March 5, 2026. As of today’s visit, the Administrator has not submitted a Special Incident Report (SIR) regarding the incident. LPA previously advised the Administrator on March 10, 2026, and again on March 12, 2026, of the requirement to submit a SIR to the Department.

Due to the facility’s failure to submit the required Special Incident Report, a citation is being issued during today’s visit.

An exit interview was conducted. Copies of the report and Appeal Rights were provided to Administrator Robert Williams.
