Complaint Investigation Report
Allegation: Staff inappropriately grabbed a client's arm while in care. During this investigation, staff interviews revealed that S-3 worked at this facility for a couple of weeks and left as S-3 was returning to school. Interviewed staff indicated they have not witnessed nor received any reports from anyone in regards to S-3 allegedly reaching behind C-2 and grabbing C-2’s hand to take away a cup of noodles C-2 was holding and stating that S-2 could not eat it while dropping it to the floor. Staff interviews also revealed that staff are trained in client rights, mandated reporting and client rights. (1) of out (3) interviewed clients indicated they did not know S-3. (2) of (3) interviewed clients indicated that they have not witnessed any staff inappropriately grabbing any clients arm. Interviews do not corroborate this allegation.
Allegation: Staff denied a client from eating while in care During this investigation, interviewed staff indicated they have not witnessed nor received any reports from anyone in regards to S-3 denying clients from eating. Staff interviews also revealed that staff are trained in client rights, mandated reporting and client rights. (1) of out (3) interviewed clients indicated they did not know S-3. (2) of (3) interviewed clients indicated that they have not witnessed any staff denying clients from eating.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
