Complaint Investigation Report
Staff were not aware of this allegation until the Adult Protective Services called and the police came to the facility. It was indicated that C1 never informed the facility staff about this occurrence. LPA interviewed two (2) clients. C1's statements were inconsistent for this allegation. The other client interviewed had not been hit or seen a staff hitting the clients. Based on information gathered, there is insufficient evidence to corroborate this allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.
An exit interview was conducted with Staff Julius Jose. A copy of this report, along with the appeal rights, was provided.
