Facility Evaluation Report

Licensing Program Analysts (LPA) Gabriela Castro conducted an unannounced required annual visit using the Compliance and Regulatory Enforcement (CARE) Tool. LPAs were greeted by Adeniran Jose, House Manager, and explained the purpose of the visit.

The facility consists of a single-story home with four (4) bedrooms, two (2) bathrooms, kitchen, dining area, living room, and attached garage. The facility is licensed for four (4) ambulatory clients. All clients residing at the facility receive case management services through San Gabriel/Pomona Regional Center.

Personal Rights postings were observed in a common area. Residents had access to personal space, privacy, and adequate storage. No firearms/weapons were present.

The facility is a one-story residential home consisting of four (4) client bedrooms, kitchen, dining room, living room, two (2) bathrooms, attached garage, laundry room, and a backyard patio. All four bedrooms contained the required furnishings, including a bed with a clean mattress, linens, dresser, and adequate lighting. Extra clean linens and towels were stored in hallway cabinets, and all furniture observed was in good condition.Both bathrooms were equipped with working sinks, toilets, and showers with non-skid mats. Hot water measured within the required range of 105°–120°F. Smoke and carbon monoxide detectors were tested and found operational, and a charged fire extinguisher were observed and available. Exits were observed to be clear and unobstructed, and no bodies of water were present on the premises.

The backyard provided shaded seating for clients. Chemicals and cleaning supplies were secured in a locked closet near the front entrance.
