Complaint Investigation Report
Allegation : Staff spoke to client in an inappropriate manner. It has been alleged that S-1 was yelling at C-1 and making fun of C-1 and C-1’s family. Interviewed staff indicated that they have not witnessed nor received any complaints from anyone regarding staff (including S-1) yelling at clients (including C-1) nor making fun of clients and/or the clients’ families. Staff interviews revealed that staff are trained in mandated reporting, zero tolerance and client rights. Interviewed staff indicated that C-1 has a history of fabricating stories and that staff are monitoring, tracking and reporting this behavior to C-1’s behaviorist. (2) out of (3) client interviews revealed that they have not witnessed any staff (including S-1) yelling at clients (including C-1) nor making fun of clients and/or the clients’ families. (2) out of (3) client interviews revealed that clients do not have any concerns regarding this matter. Interviews do not corroborate this allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
