Complaint Investigation Report
Allegation: Facility staff forcefully grabbed client. It is alleged that S-1 forcefully grabbed C-1 by their left upper arm and requested C-1 to wash a dirty dish that was left in the sink. C-1 allegedly refused to wash the dish and then S-1 released C-1’s arm with no injuries noted. Staff interviews revealed that staff do not forcefully grab clients. Staff interviews also revealed that they have not witnessed nor received any complaints/concerns from anyone in regards to staff forcefully grabbing clients. Staff interviews revealed that C-1 has a history of fabricating stories. Interviewed staff indicated they are trained in mandated reporting, client rights and zero tolerance. (1) out of (2) interviewed clients indicated staff do not forcefully grab clients. Staff and client interviews do not corroborate this allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
