Facility Evaluation Report

Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Michael White and discussed the purpose of today’s visit.

This is a two-story home. The first floor consists of a living room, office, dining room, kitchen, (1) bathroom and laundry room. The second floor consists of (3) bedrooms and (1) bathroom. This home is approved for (4) ambulatory clients. San Gabriel Pomona Regional Center provides case management services for all clients residing at this home. A plan to have surveillance cameras in common areas has been approved. Placement of surveillance cameras observed to be in common areas as noted on the approved plan.

LPA utilized the Compliance and Regulatory (CARE) tools for the visit today and observed the following:

Infection Control: Facility has an infection Control plan in place.

Operational Requirements: Staff are adhering to operational requirements.

Physical Plant & Environment Safety: Carbon monoxide detectors were tested and operable (downstairs and upstairs). The smoke detectors were tested and operable (downstairs and upstairs). The fire extinguishers were observed and appeared to be full (last service date of 03/25/25). Hot water temperature measured at 114.2* in the bathroom downstairs and 115.1* in the bathroom upstairs. Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. Passageways were free of obstruction.
