The inspector’s account

Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced annual inspection utilizing the Compliance and Regulatory Enforcement (CARE) Tool. Upon arrival, the LPA was greeted by Direct Support Professional (DSP) Maureen Onyiaekwuazi, who was informed of the purpose of the visit. Administrator Ifeyinwa Olatilewa arrived shortly thereafter.

The facility is a single-story home located in a residential neighborhood. All clients residing in the facility receive case management services through the San Gabriel/Pomona Regional Center. The facility is licensed to serve four (4) ambulatory clients between the ages of 18 and 59.

The garage area is not approved for use as a living space.

The facility is a single-story residential home consisting of four (4) bedrooms, a kitchen, dining room, living room, two (2) bathrooms (one of which is designated as a private bathroom), a laundry area, and a backyard patio. The backyard patio is furnished with patio furniture and equipped with an umbrella/canopy to provide shade. All four (4) client bedrooms were observed to contain the required furnishings, including a bed with a clean mattress, linens, a dresser, a chair, and adequate lighting. Additional clean linens and towels were readily available in client rooms, with extra supplies stored in a hallway closet and an unoccupied bedroom. All furniture was observed to be in good condition. The bathrooms were equipped with functioning fixtures and non-skid mats. Hot water temperature was measured and found to be within the required range of 105°F to 120°F. Smoke detectors and carbon monoxide detectors were present and operational. A fully charged fire extinguisher was properly mounted in the kitchen area. All exits were unobstructed, and no bodies of water were observed on the premises.
