Complaint Investigation Report
He stated the staff are trained to handle clients with care. They are professional and do not lay hands on clients. Staff interviewed denied slapping any clients on the head and have not observed any staff doing so. LPA interviewed 3 Clients who stated that staff do not hit them or other clients.
Allegation – Staff threatened to pour water on client. LPA interviewed 6 staff members who all denied filling a cup of water, chase Client #1, and threatened to pour the water on client. They have not seen any other staff threatening clients. They stated they redirect clients when necessary and client will comply. They receive ProAct training and will redirect a client before any behaviors escalate. They do not threaten clients to get them to comply either. LPA interviewed 3 Clients and 2 stated that staff have not threatened to pour water on anybody. One of the clients did not provide a response.
Allegation – Staff did not treat client with dignity or respect. Administrator stated that the staff are trained professionals and are often reminded of the client rights. They are patient with the clients and assist them when needed. Staff interviewed stated they treat clients with dignity and respect. They have not seen other staff yelling at clients. They remain calm when a client exhibits a behavior to prevent it from escalating. They will explain things to them and allow them to make their own choices. Staff indicated they receive training on client rights and ways to handle clients appropriately. LPA interviewed 3 clients and one of the clients did not respond to the questions being asked. 2 out of the 3 clients interviewed like the staff and are treated with respect. They stated staff do not yell at them or other clients.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
An exit interview was conducted with the administrator. A copy of this report along with the appeal rights were provided.
