Complaint Investigation Report
Allegation: Facility staff do not speak to client with dignity and respect. It has been alleged that staff do not speak to clients with dignity and respect. Staff interviews revealed that staff treat and speak to all clients with dignity and respect. Interviewed staff indicated that staff (including S-1) do not bully nor speak poorly to any clients (including C-1). Interviewed staff indicated that they have not received any complaint nor concerns pertaining to this matter. Interviewed staff indicated that they are trained in client rights, mandated reporting and zero tolerance. Client interviews revealed that staff treat clients with dignity and respect. Interviewed clients indicated that staff (including S-1) do not bully nor speak poorly to any clients (including C-1). Interviewed clients indicated that they have not heard anyone complaining about this matter. Interviewed clients indicated that they have not witnessed any staff being disrespectful to any clients. Client interviews revealed that they are happy residing at this facility and they do not have any concerns. Interviews do not corroborate this allegation.
Allegation: Client was not permitted to stay home from day program. It has been alleged that clients are not permitted to stay home from day program. Staff interviews revealed that clients are permitted to stay home from day program. Interviewed staff indicated that (2) out of (4) clients attend day program services at this facility (4) to (5) days per week. Interviewed staff indicated that the other (2) clients attend a traditional day program outside this facility. Staff interviews revealed that all clients are permitted to stay home from program (including clients attending program at this facility). Interviewed staff indicated that clients are not forced to attend/participate in day program. Interviewed staff indicated that they have not received any complaints nor concerns pertaining to this matter. Client interviews revealed that clients are allowed to stay home from day program. Client interviews revealed that they can also select not to participate in program and stay home. Interviewed clients indicated that staff do not force clients to attend nor participate in day program. Interviewed clients indicated that they have not heard anyone complaining about this matter and they do not have any concerns. Interviews do not corroborate this allegation.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
