The inspector’s account

No staff were physically present to observe the incident. When interviewed, C2 denied threatening C1. Interviews overall show that staff spoke with both C1 and C2 about the situation and worked on an agreement when it comes to the facility thermostat. Interviews do not show that staff ignored C1. Review of staff notes show that on 6/14/22 C1 and C2 were adjusting the facility thermostat. No notes observed of the clients threatening each other. Based on the interviews conducted filed reviewed, there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit Interview conducted with Andrew Devera and a copy of this report was provided.
