Complaint Investigation Report
LPA interviewed three (3) staff S#1 -S#3 and all three staff denied the allegation. LPA interviewed ten (10) residents and nine (9) of ten (10) residents could not corroborate the allegation. All three (3) staff stated that C1 is not being evicted. Staff stated that they have moved C1’s roommates at least five (5) times to other rooms due to C1 complaints about them. But that C1 has never brought up the allegations that roommate was abusing C1 mentally. Nine (9) of ten (10) clients interviewed stated that they have never being harassed by C6 and have never heard or witnessed C6 harassing anyone. LPA took tour of facility and did not observe any clients harassing any other clients and all were minding their own business and calm.
Based on statements and interviews conducted with staff, clients, and review of client file, there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED .
