Complaint Investigation Report
Records showed that C1’s lab work was last completed in March 2025. There were no lab reports from April to August observed in the file. Staff could not confirm if the monthly lab work had been done. Staff stated C1 was not feeling well and was taken to the hospital on 9/9/25 and again on 9/12/25. The hospital determined the client had Lithium Toxicity. Other staff interviewed were not sure if C1 needed to get monthly lab test for Lithium medication. LPA interviewed C1, who was aware that a monthly blood test was needed but could not recall the last time it was done. C1 knows the importance of maintaining good health and would never refuse the lab test. Two (2) other clients interviewed, who need lab work done monthly, stated they get their blood drawn at the facility. The other nine (9) clients interviewed do not require any specific medical needs.
Based on interviews conducted and record review, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6 and Chapter 6), are being cited on the attached LIC 9099D.
An exit interview was conducted. The Plan of Correction was reviewed and developed with Staff J. Elias. A copy of this report and appeal rights were provided.
