Complaint Investigation Report
As an infection control measure, the facility served meals on plastic plates that can be disposed of after each use. LPA observed multiple trash can throughout the dining and kitchen area. Interviews with clients show that staff have not been observed reusing anyone's leftover meal to prepare another clients meal. One (1) client interviewed believes staff may be reusing food but had not observed it. Based on statements and interviews conducted with staff and clients as well as LPA observation, there was not enough supportive evidence to concur with the reported allegations. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Exit interview was conducted with Yitzi Teichman and Julia Elias. A copy of this report was provided.
