The inspector’s account

Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced case management visit in order to do a health and safety check on the facility following a Serious Incident Report (SIR) that was received on 4/7/2025 involving Client #1 (C1) starting a fire on the front patio near the main entrance of the facility. LPA met with Joyce Garcia, administrator of the facility, and explained the purpose of the visit.

Joyce explained that the fire had started after C1 had lit a towel on fire and threw it onto a patch of grass that is near the front entrance of the facility. Joyce explained that staff were able to quickly extinguish the flame and that it did not grow to be large in size. Joyce stated that no clients were injured because of the fire and that there was no structural damage to the facility due to the fire. Additionally, Joyce explained that none of the facility operations have been affected because of the fire including being able to make meals for the clients, being able to distribute medications to the clients, and none of the water has been affected or contaminated.

During today's visit LPA toured the facility including the area that was set on fire during the incident, which is about ten (10) yards away from the main entrance of the facility and did not affect the building. No health or safety concerns were noted during the visit.

No deficiencies were observed during today's visit. Exit interview was held and a copy of the report was provided.
