Facility Evaluation Report

Licensing Program Analyst (LPA) Galarza generated this Case Management - Deficiencies report in conjunction with complaint control # 28-AS-20220705084756 pertaining to observations made during the kitchen inspection. The purpose of the visit was explained to Med-Tech Fabiola Padilla. Supervisor Andrew De Vera assisted with the inspection.

The kitchen refrigerator temperature was 62 degrees instead of a minimum of 41 degrees per state law temperature requirements.

The right stove has 6 burners, of which 3 burners were not working properly.

Seven (7) out of 10 burners on the left stove do not work when the knob is turned on.

There was gas odor in the kitchen. The Gas Company was called and visited the facility today.

Six (6) out of the 16 stove burners had flame without being turned on. Staff stated they have been this way for over 1 year.

An exit interview was conducted and a copy of the report and appeal rights were issued to Fabiola Padilla.
