Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez and Licensing Program Manager (LPM) Adeline Ho conducted an initial unannounced complaint visit to investigate the allegation listed above. LPA and LPM met with Andrew De Vera, Assistant Administrator and explained the purpose of the visit. Kenyada Melton, maintenance, assisted with the facility tour.

During facility tour with Kenyada, LPA observed in the hallway, outside room #11, there is a missing ceiling panel that shows exposed wiring and pipes that poses a health and safety concern.

An exit interview was conducted, and a copy of this report was provided to Andrew De Vera Assistant Administrator. Appeal rights also provided and discussed.
