Complaint Investigation Report
During interviews with the clients, none of them corroborated the allegation. C1 had stated that they have not been physically harmed by any of the clients or staff recently. All of the other clients interviewed stated that they have never been hurt physically by anyone else while living in the facility. During interviews with the staff, none of them corroborated the allegation. One of the staff interviewed stated that no one in the facility has harmed C1 at all recently. Another staff member stated that they have not heard of C1 being harmed by anyone in the facility, and that nothing like this has been reported to staff. LPA did not observe any injuries to C1's body during the interview.
In regards to the allegation that " Staff did not safeguard client's money," it is alleged that someone at the facility has been taking C1's money. During interviews with the clients, none of them corroborated the allegation. C1 did not mention that their money has been taken from them by anyone during the interview, and stated that they are working towards having the facility become their own payee for social security. All other clients denied having any problems with their money or it being taken from them by anyone while living in the facility. During interviews with the staff, none of them corroborated the allegation. One of the staff interviewed stated that C1 is currently their own payee, so the facility is not even able take the money of C1 in any way. All other staff stated that they have never heard of concerns regarding C1's money being taken from them.
Based on statements and interviews conducted with staff, clients, review of client files and facility file records, there was not enough supportive evidence to concur with the reported allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED .
