Complaint Investigation Report
Interviews conducted with S1-S2 revealed that they were not aware of the incident and did not witness the alleged incident. 3 out of 5 staff stated that they heard about the incident from another staff but they did not witness the alleged incident. Staff members interviewed indicated that if they witness an altercation between clients they will attempt to stop the altercation and will notify facility management immediately. Facility staff will also contact Law Enforcement and seek medical attention for clients if needed. None of the clients interviewed indicated that they did not witness the alleged incident between C1 and C2. Clients interviewed indicated that they feel safe and comfortable at the facility and confirmed that facility staff will intervene during client-on-client altercations. Therefore, there was insufficient evidence to corroborate with this allegation.
Based on statements and interviews conducted with clients and staff as well as reviewed files, there was not enough supportive evidence to corroborate the allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
