Complaint Investigation Report
Based on the interviews and observations conducted there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
In regards to the allegations "Staff failed to keep the facility clean." it was alleged that C2 dirties their restroom with feces and the staff do not clean it. (7) of (7) Staff interviewed denied the allegation. (15) of (16 ) clients interviewed could not corroborate the allegation. Interviews show that staff regularly clean every clients room. Each room has an attached private bathroom. If there is an issue, clients will let staff know and it is addressed in a timely manner. Staff interviewed did not state to be aware of issues regarding C2 leaving their bathroom sink and toilet covered in feces. C2 was interviewed and denied leaving their restroom dirty. LPA observed C2's bathroom, attached to room #46 , and did not observe it to be dirty or have feces spread around.
