Facility Evaluation Report

Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced case management visit in response to Serious Incident Report (SIR) dated 5/12/2025 that describes the alleged inappropriate touching of Client #1 (C1) by Staff #1 (S1). LPA met with Chimezie Okereke, caregiver for the facility, and explained the purpose of the visit. Administrator Lan Ly was notified of the visit by phone call.

LPA Interviewed administrator Lan Ly by phone regarding the incident. Lan explained that on 5/12/2025 C1 made the allegation that S1 had touched them inappropriately in their private area while S1 was taking them to the mall. Lan explained that this had followed an incident on 5/3/2025 in which C1 had fallen while out on a walk with S1, and Lan advised S1 not to go on outings with C1 without another staff member supervising them. Because S1 had broken this agreement by once again taking C1 on an outing alone on 5/12/2025 when they took C1 to the mall and which C1 made the allegation that they were touched inappropriate, S1 was terminated on 5/14/2025. APS and law enforcement also made visits in response to the incident, however they have not verified or confirmed the allegation made by C1, and Lan explained that S1 was terminated a precaution for the health and safety of the clients. LPA spoke with C1 during the visit, and C1 denied that any incident had occurred between themselves and S1.

LPA asked administrator to email C1's FACE Sheet, Most Recent Physician's Report, most recent IPP, behavioral notes, along with the FACE Sheet and health screening of S1. LPA also requested the APS and law enforcement report numbers from the administrator as well.

Following the visit, no deficiencies were noted. Exit interview was conducted and a copy of the report will be emailed to the administrator.
