Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit in relations to a complaint investigation. LPA met with Staff, Isidro Serrano, and explained the reason for the visit.
During a visit held on 3/7/25, LPA conducted a walk through of the facility and observed a hole behind the poster of the kitchen wall. The administrator who was present at the time of visit was not aware of the hole. A client and a staff knew about the hole a while back but did not report it.
A deficiency was issued today. A copy of this report along with appeal rights were given to Staff Serrano.
