Facility Evaluation Report
Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Jennifer Miano and discussed the purpose of today’s visit. Leah Avila (Administrator) arrived at approximately 8:20 A.M..
This is approved for (6) ambulatory residents. This home consists of (3) bedrooms, (2) bathrooms (1 of which is in between room #2 and room #3), (1) staff bedroom, kitchen, dining room, living room, laundry room and a detached garage. Frank D. Lanterman Regional Center provides case management services for all clients residing at this home.
LPA utilized the Compliance and Regulatory (CARE) tools for the visit today and observed the following:
Infection Control: Facility has an infection Control plan in place.
Operational Requirements: Staff are adhering to operational requirements.
Physical Plant & Environment Safety: The smoke and carbon monoxide detectors were tested and all are operable. The fire extinguishers were observed (kitchen and living room) and appeared to be full (last service date of 04/25/25). Hot water temperature measured at 118.8 in the hallway bathroom and 117.3* in the bathroom between bedroom #2 and bedroom #3. Knives, cleaning solutions, and disinfectants are locked and inaccessible to clients. Passageways were free of obstruction.
