Licensing Program Analyst (LPA) Mary Flores conducted an unannounced collateral visit at the facility to conduct interviews regarding a recent incident at a different licensed facility. LPA met with Marie Brooks and Kay Cano Administrator and explained the reason for the visit.
The purpose of this visit was to conduct interviews with 7 residents regarding the incident that occurred at the residents' previous facility.
Exit interview was conducted with administrator and a copy of this report was provided.
