Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Case Management – Deficiencies visit today. LPA met with Carolee Hazzard, Administrator, and the reason for the visit was explained.

During today’s visit, citations will be reissued for uncleared Plan of Corrections (POCs) which were due on 6/30/2026 for the following sections:

CCR Section 85064(b) – Administrator Qualifications and Duties. Staff have not renewed their administrator certification.

CCR Section 85064(k) - Administrator Qualifications and Duties. Administrator does not have HIV and TB training.

During today’s visit, deficiencies were note and citations were issued. Exit interview conducted with Carolee Hazzard and copy of this report, LIC 809-Ds and Appeal Rights was provided.
