Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management Visit-Deficiencies today, stemming from initial complaint investigation conducted on 7/17/2026. LPA met with Carolee Hazzard, Administrator, and the reason for the visit was explained.

LPA Cota, attempted to review file for P1 during complaint investigation visit; however, S1 could not locate the file.

Deficiency is being cited. Exit interview was conducted with Carolee Hazzard, Administrator, and a copy of the report, LIC 809-D and Appeal Rights was provided.
