Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Case Management Visit – Plan of Correction (POC) to follow up on deficiencies cited during annual visit conducted on 4/23/2026. LPA met with Carolee Hazzard, Administrator, and the reason for the visit was explained.
During today’s visits, LPA assessed Civil Penalties for FAILURE TO CORRECT for the following:
HSC Section 1565(a): Other Provision. Licensee does not have an Emergency and Disaster Plan (LIC 610-D) in place. Civil Penalty has been assessed for $700.
CCR Section 80087(a): Building and Grounds. Licensee did not conduct clean/removal of all grease and mildew on the stove, walls, and hood vent. Civil Penalty has been assessed for $250.
The following deficiency was corrected during today’s visit:
CCR Section 80075(f): Health-Related Services. Licensee provided LPA proof of First Aid/CPR training appointment for (6) out of (6) staff scheduled to take place on 5/29/2026.
Exit interview was conducted with Carolee Hazzard, Administrator, and copies of the report, POC letter, civil penalty forms and Appeal Rights were provided.
