Facility Evaluation Report

On 5/12/2022, Licensing Program Analysts (LPAs) Bennette Pena and Elizabeth Irra conducted an unannounced case management visit to follow-up on the death report of Client 1 (C1). LPAs met with Administrator, Ruby Garcia and explained the reason for the visit. LPAs reviewed the files and obtained relevant documentation for C-1.

Cause of death is unknown currently. Administrator will provide a copy of C1's death certificate to LPA B. Pena when it becomes available.

LPAs toured the facility grounds and did not observe any health and safety concerns.

Exit interview conducted and copy of Report was provided to Administrator, Ruby Garcia.
