Facility Evaluation Report

Licensing Program Analyst (LPA) Bennette Pena conducted a subsequent visit to continue and complete the required 1-yr inspection. LPA was met by Rose Grant, DSP and explained the purpose of the visit. LPA & DSP spoke with Administrator Dwayne Lundy on the phone and shortly after, administrator arrived at the facility. The inspection was stopped because LPA faced harassment from a hostile and aggressive administrator. LPA felt threatened by the administrator's behavior, which included shouting and slamming heavy binders on the table. As a result, LPA felt the need to leave the facility after reporting the incident to LPM D. Sicairos.
