Facility Evaluation Report

Licensing Program Analyst(s)(LPA) Sanjay Vaid conducted an unannounced annual visit at the facility using the CARE inspection tool. LPA Vaid met with Carlos Sanchez Direct Support Staff (DSP) and Heidy Flores (DSP) and explained the reason for the visit. Melvin Alvarez arrived 30 minutes later.

The facility is licensed to serve four (4) Developmentally Disabled Clients ages 18 - 59 years. The facility is approved for two (2) Non-Ambulatory Clients. The facility a single story and is located in a residential area. A tour of the facility includes: Four (4) client bedrooms, two (2) bathrooms, living room, kitchen, dining area, and indoor/outdoor activity areas.

LPA conducted a tour of the facility with Carlos Sanchez and observed the following.

Facility's living room has sufficient sitting space, no fireplace was observed. Kitchen was observed clean and in working condition. Food supplies were observed sufficient for at least (2) days of perishables and (7) days of non-perishables. Medication and sharps cabinet was observed in hallway closet and locked. Passageway to the backyard was observed clear. Lock cabinet for cleaning supplies, sinks accessible to the clients, water temperature was measured at 105-120 degrees F. Bedrooms #1-#4 (BR1-BR4) and were observed to have the required furniture and bedding supplies. Front porch and backyard have shaded sitting area available. Smoke/Carbon monoxide detectors were tested and working. Fire extinguisher was observed and last checked on 04/23/2025. Medication and files were checked for 4 clients and 3 staff files were checked. Emergency disaster plan was reviewed, last fire drill is conducted 4/7/25 last earthquake drill was held in 4/12/2025. Administrator Certificate for Melvin Alvarez Mendoza- 2/17/26 and Alicia Walters expires in 10/02/2026. Interviews with three staff, clients are non-verbal.

No deficiencies were noted, exit interview was conducted with Melvin Alvarez Manager and a copy of this report was provided.
