Facility Evaluation Report

Licensing Program Analyst, (LPA), Mayra Cota, conducted a Plan of Correction (POC) visit today. LPA met with Lerisa Canto, House Manager and explained the reason for the visit. Administrator, Carol Liess was informed of the visit via phone call and reason for the visit was also explained.

During today's visit, a citation was cleared for deficiency cited in Section CCR 85064(f) which was issued on 6/10/2025. The deficiency was cleared by means of obtaining updated LIC 308 (Designation of Facility Responsibility) from licensee by POC due date.

Amended LIC 809 (Facility Evaluation Report) and (Facility Evaluaiton - Deficiencies Report) LIC 809-D with correct POC date from visit conducted on 6/10/2025 was discussed with House Manager and signed.

As of 6/13/2025, LPA has not received the LIC 200 and complete facility sketch including the ADU by POC due date, and therefore, a Civil Penalty was assessed today for $300 for Failure to Correct for Section 80086(a).

Exit interview conducted with Lerisa Canto, House Manager and copies of this report, amended LIC 809 and LIC 809-D, Letter of Deficiency Citations Cleared, Civil Penalty Assessment - Failure to Correct, and Appeal Rights provided at the time of visit.
