Facility Evaluation Report

Licensing Program Analysts (LPAs) Mayra Cota and Blanca Gonzalez, conducted a Plan of Correction (POC) visit. LPAs met with Lerisa Camtos, House Manager and explained the reason for the visit. Carol Liess, Administrator, was contacted telephonically by LPAs during visit and the reason for the visit was also explained.

During today's visit, the following was discussed with House Manager and Administrator:

A previous licensing report was issued on 5/22/25 giving notice of violation of CCR Section 80086(a) due to licensee not notifying the department of construction, changing walls inside the home, and adding an ADU in the garage. Because the citation was not corrected due to licensee submitting documents which were incomplete by Plan of Correction (POC) due date, Civil Penalty was assessed - Failure to Correct for period of 5/30/25 to 6/10/25 for $1,200.

Exit interview conducted with Lerisa Cantos, House Manager and a copy of this report, Appeal Rights, and LIC 421FC (Civil Penalty - Failure to Correct) was provided and emailed to Administrator, per request during time of visit
