Facility Evaluation Report
Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required- 1 year visit. LPA was met by Dan De Jesus and Dolly De Jesus, Homeowners and explained the purpose of the visit. LPA was informed by the homeowners that, to their knowledge, the facility has shut down and that Ralph Estanislao (Administrator) , the previous renter, has been skipping rent payments since September 2025. According to the homeowners, they moved back to the home in November 2025 and the Administrator left the house unkempt and in a state of disrepair.
LPA did not utilize the Compliance and Regulatory Enforcement (CARE) tools for the visit today. LPA conducted a tour of the facility to ensur e that there are no residents in the home needing care and supervision. Currently, the facility has zero (0) residents under care and supervision and zero (0) staff present. The facility is a single story home consists of (5) bedrooms, (3) bathrooms, a living room, a dining room, a TV room, a kitchen and an attached garage. The outdoor facility has a front yard, and back patio.
At 12:30pm, LPA left the Administrator a voicemail message requesting the Administrator to call LPA back to discuss the closure plan and provide the documents needed to submit. After reviewing the facility's history, LPA did not find any communication reported between the Administrator and CCLD regarding the facility's intent to close nor facility's closure plan.
No exit interview conducted and no deficiencies cited at this time. A copy of this report will be emailed to the Administrator, Ralph Estanislao.
