Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management (CM) – Deficiencies visit today. LPA met with Rosalie Sandoval, Director and the reason for this CM visit was explained.
On 2/17/2026, LPA Cota attempted to review records for Resident 1 (R1) for complaint investigation #28-AS-20260212100512; however, staff were unable to provide LPA with Medication Sheets prior to January 2026 and PRN Medication Logs prior to 10/8/2024, even though R1 had been living in the facility since 10/11/2018. Interview with S2 and S3 indicated that as of 2/2/2026 R1 is no longer living in the facility but could not locate R1’s Medication Sheets and PRN Medication Logs. The fore mentioned documents were not readily available for review during the visit.
Citation is issued for CCR Section 87506(a) Resident Records (a) The licensee shall ensure that a separate, complete, and current record is maintained for each resident in the facility or in a central administrative location readily available to facility staff and to licensing agency staff.
During today’s visit, a citation was issued. A copy of this report, LIC 809-D and Appeal Rights was issued. Exit interview conducted with Rosalie Sandoval, Director.
