Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management (CM) – Deficiencies visit during a complaint investigation conducted today. LPA met with Rosalie Sandoval, Director and the reason for this CM visit was explained.
During today’s visit, LPA conducted an audit of Resident 5 and Resident 6 (R5 and R6) routine and PRN medication and observed expired bubble packs as follows:
Acetaminophen 325 mg. tablets to be taken every (4) hours or as needed expired on 10/14/2025.
Guaifenesin-DM100-10 mg to be taken every (6) hours or as needed expired on 11/22/2025.
Ibuprofen 400 mg tablets to be taken every (8) hours or as needed expired on 12/27/2025.
Acetaminophen 325 mg tablets (2) tablets to be taken (=650 mg) every (4) hours or as needed expired on 12/27/2025.
Lorazepam 1 mg tablet to be taken twice a day or as needed expired on 12/14/2025.
During today’s visit, a deficiency was noted and a citation was issued. A copy of this report, LIC 809-D and Appeal Rights was issued. Exit interview conducted with Rosalie Sandoval, Director.
