Facility Evaluation Report
Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA was met by Matthew De Vera, Administrator and explained the purpose of today's visit. The facility is approved for four (4) ambulatory developmentally disabled adults, age range 18-59.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Staff are adhering to infection control requirements. The facility has an Infection Control Plan.
Operational Requirements: A fire clearance is in place. LPA observed the Infection Control Plan in place. LPA observed Liability Insurance and Surety Bond that are valid and in place. Last fire drill was held on 01/21/2026.
Physical Plant & Environment Safety: This facility is a single-story home consists of four (4) client bedrooms, three (3) bathrooms, laundry area, kitchen, dining room, living room and a shaded patio with seating. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Extra linen and towels are in the laundry closet. Smoke alarms and carbon monoxide were tested and operable. Laundry area is next to the bathroom. There are two (2) fire extinguishers mounted on the wall in the kitchen area and laundry area which were purchased on 02/04/2026.
