Facility Evaluation Report
Licensing Program Manager (LPA) Mary Flores conducted an unannounced case management visit at the facility to follow up on incident report submitted to the department on 4/1/25. LPA met with Joey Perez and explained the reason for the visit.
On 4/1/25 facility submitted an incident report to the department to report that on 3/29/25 staff #1(S1) had entered a client's room and used an unwanted tone of voice.
On 4/17/25 LPA Flores conducted a case management visit and conducted interviews with 2 clients, 2 staff, and attempted interviews a staff over the phone.
LPA reviewed the following documents; Staff #1's file, documented incidents dated 10/17/24, 10/20/25, 12/4/24. Each report was of the rapport staff had built with the clients in care and how to improve it. On 4/1/25 a statement of a meeting with Staff #1 and 2 additional staff was taken regarding the incident report. Per statements reviewed S1 intervening in during a de-escalating situation with client #1 in an unprofessional way by making statements such as "stop acting like a baby" and "shut up". Administrator conducted an internal investigation. Lanterman Regional Center conducted an investigation on 4/4/25 and notified the administrator to have unsubstantiated the incident. However, suggested that a plan be in place if the staff continue to provide care. Prior to the incident administrator attempted to mediate the situation as it had not been to the extend of the day of the incident.
During this visit LPA found that S1 used coarse language towards a client in care. Therefore is noting a deficiency on LIC 809D per Title 22 Regulations.
Exit interview was conducted with Joey Perez and Ernesto Vasquez Administrator and a copy of this report was provided.
