Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual visit today. LPA met with Ernesto Vasquez, Program Manager, and the reason for the visit was explained. Administrator, Saundra Sahagun, arrived thereafter and continued to facilitate the visit.
The facility is licensed to serve (4) non-ambulatory adults ages 18 to 50 years of age. Clients in care receive services through Frank D. Lanterman Regional Center. There are currently (3) clients living in the home.
The home is in a residential area of Pasadena. The home consists of living room, dining room, kitchen, office, (4) client bedrooms, (2) full and (1) half bathrooms, attached garage with laundry area, front and backyard with a shaded patio area.
The following was observed during today’s visit:
Facility is clean and walkways, passages and stairwell are free of debris and obstructions. Furniture throughout the facility was observed in good repair and there is sufficient seating for clients. Client bedrooms have the required furniture such as bed frames, dressers, lamps, and chairs.
Bathrooms were inspected and observed clean and sanitary. Bathrooms have safety grab bars and anti-slip mats. The water temperature was tested in the bathrooms and measured within 105.3 – 114.2 degrees F. which is within the required compliance range of 105 - 120 degrees F. Sufficient personal hygiene supplies for clients are available for use. Laundry appliances observed to be working properly, and detergents are kept locked in a cabinet in the garage.
