Facility Evaluation Report
Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA was met by the Administrator, Millet Pineda and the LPA explained the purpose of the visit. The facility is licensed to serve four (4) Non-Ambulatory Adults ages 18-59 years. The facility currently has two (2) clients that are serviced through Frank D. Lanterman Regional Center.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Facility maintains the required Infection Control Plan. Staff are adhering to infection control requirements.
Physical Plant & Environment Safety: The facility is a two-story home located in a residential area in Pasadena, Ca. only the first floor of facility is licensed to provide care. A tour of the facility includes: two (2) client bedrooms, two (2) bathrooms, living room, dining area, kitchen, laundry area, detached garage, front yard, back yard and locked shed used for storage. LPA toured facility, clients’ bedrooms were checked and closet/drawer space to accommodate each client comfortably was available. The backyard is free of debris/hazards and the outdoor and passageways are free of obstruction. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. Hygiene products, Extra linens and towels are readily available for clients. The fireplace is closed and inaccessible to clients. All storage areas for cleaning solutions, toxins, knives, and hazardous items are kept in a locked cabinet and are inaccessible to clients. [Continue to LIC809-C]
