Facility Evaluation Report
Licensing Program Analyst(LPA) Sakinah Madyun and Out Of Class-Licensing Program Manager(LPM) Mary Flores conducted a Case Management visit regarding capacity increase request submitted to the Department on 6/3/25. LPA and LPM met with Jose Garcia and explained the reason for the visit.
The home is currently licensed to serve (14) Ambulatory clients between the ages of 18-59. The facility is located in a residential area and consist of a two level home with a living room/dining room area, a kitchen, a backyard, (8) bedrooms, (3) bathrooms, a front porch, a detached garage, and a shed out back.
No changes have been made to the structure of the home. The capacity increase will take place by changing an office space into a single client bedroom. The Fire Clearance was granted on 6/3/25 for (15) Ambulatory clients between the ages of 18-59.
During today's visit LPA and LPM conducted a tour with Caregiver/Staff Jose Garcia and observed the following:
Physical plan observed on the first floor a living room/dining, (1) staff bathroom, (1) client bathroom, (1) staff room, a kitchen, and (3) client bedrooms. One of the client rooms was filled with office equipment. The second floor was observed with (5) client bedrooms and (2) bathrooms. Bedroom #8 is a shared room divided with a wall and door exiting to another clients room. Per the facility sketch there should be a closed wall. Water temperature was tested in each bathroom between 115.3-118.0 degrees F., which is within the required 105-120 degrees F.
