Complaint Investigation Report
During interviews with the clients, five (5) out of six (6) did not corroborate the allegation. One client stated that they have not had their laundry come back dirty after using the washer and dryer in the facility. Another client interviewed also stated that their laundry comes back clean every time they use the washer and dryer. During interviews with the staff, none of them corroborated the allegation. One staff interviewed stated that they ensure that the washer and dryer are always clean by utilizing bleach to make sure that clothing and blankets do not come back dirty. Another staff also stated that no one in the facility is adding dirt into the laundry when clients beddings and clothes are being washed. During a tour of C1's room, LPA did not observe any dirt or stains on the bedsheets for the client.
Based on interviews conducted and records reviewed, there is insufficient evidence to support the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
An exit interview was conducted, and a copy of this report was emailed to Administrator due to LPA having printer problems.
