Facility Evaluation Report
Licensing Program Analyst (LPA) Tao conducted an unannounced annual visit at the facility using the CARE inspection tool. LPA met with A dministrator Desiree Alvarado and the reason for the visit was explained.
This facility is licensed to serve fourteen (14) ambulatory mentally ill clients between the ages of 18 to 59 years old. Facility annual fees are current. Today’s inspection consisted of applying CARE tool, conducting physical plant, reviewing staff/client records, checking client’s food supply/medication, and interviewing staff/clients.
Facility maintains an infection control plan was on file. Staff were observed practicing safe hand washing. Hand sanitizing was observed throughout. Hygiene supply and sanitizing materials were observed in each bathroom and changing station.
The facility is a two-story home located at a residential area. On the first floor, it consists of three (3) client rooms, a TV/ dining room, a kitchen, laundry area, a full bathroom for client, a half bathroom for staff, an office with access to the basement, and a staff room. On the second floor, it consists of five (5) client bedrooms and two (2) bathrooms which were one (1) full and one (1) half bathroom. ( - Continued on LIC 809C - )
