The inspector’s account

Licensing Program Analyst (LPA) Mary Flores conducted an unannounced case management visit at the facility to follow up on capacity increase request submitted to the department on 9/5/25. LPA met with Michael Barkyoumb Program Director and explained the reason for the visit.

The facility is currently licensed to serve 36 adults of which 28 may be ambulatory and 8 non-ambulatory. The facility is located in a residential area and consist of a one level building with (4) offices, (1) career office, (1) conference room, (2) staff lounges, (1) client activity room, (1) shared client/child activity room, (1) isolation room, (4) client bathrooms in the front of the facility, 1 out of the 4 has client change room, a kitchen, a flex convert client/child room, (3) outdoor spaces, (4) client/children bathrooms in the back of the facility and (1) shower/bathroom.

No changes have been done to the building. The capacity increase will take place by changing the career office into another learning area for adult clients/participants, additional staff has been hired to maintain ratio per the program design which will remain 3:1. Additional outings will be implemented, and use of other resources to commute during outings. Facility will continue to separate and ensure child care and adult day program does not commingle during their business hours.

The Fire Clearance was granted for 36 ambulatory and 12 non-ambulatory, for a total of 48 individuals on 10/1/25.

After tour of the facility there are no concerns regarding the physical plant.

Exit interview was conducted and a copy of this review was provided.
