Facility Evaluation Report

Licensing Program Analysts (LPAs) Blanca Gonzalez and Nune Margaryan conducted an unannounced Case Management Visit. LPAs met with Daniel Adeleke, House Manager and explained the reason for the visit. Administrator Omobola Adeleke arrived shortly after.

Purpose of the visit was to follow up on citations given on annual visit conducted 06/16/2025 and to provide Licensee with resources to improve with operations and complying with Title 22 regulations. All citations have now been cleared and LPAs confirmed compliance at the time of this visit.

· Reminded Licensee about regulations regarding notifying Licensing prior to any construction

· Licensee submitted updated facility sketch and was advised to not make any further changes without notifying CCLD

· Asked about client placement service. Licensee indicated clients are placed by Health Services/Department of Mental Health, not Regional Center. An application for vendorization was not completed and Licensee did not follow up.

· Licensee was advised to complete a new Plan of Operation reflecting the changes in how they receive clients and staff training requirements

· Licensee had previously requested capacity increase from 6 to 9. Licensee is not interested in continuing with the increase process and will remain at capacity of 6 clients.
